Project Safeguard Consent for Disclosure, Communication, and Release of Personal Information

Dear Veteran:

Thank you for your service in the military. We want to take this opportunity to introduce Project Safeguard, an initiative developed to rapidly and efficiently connect Veterans to earned benefits and community-based services.

If you agree to participate in the program, you will complete a short screening survey to identify areas where you might benefit from assistance, and then be connected to a Project Safeguard Community Care Navigator in your area. This Community Care Navigator can assist you with accessing earned Veteran benefits, connect you to local medical providers, and refer you to a vetted network of non-clinical service providers.

By participating in the program, you are consenting to sharing your information and any personal information we obtain with the Project Safeguard team. This allows them to conduct additional analysis of the data to help us better understand your needs and to evaluate, as well as improve, the programs provided to Veterans. Overview of Program

Project Safeguard is an innovative, no-cost program with the goal of helping Veterans connect to earned benefits and local community-based services. The program utilizes multi-organizational efforts to provide the best resources, requiring coordination conducted in the most efficient and time-sensitive method after a Veteran signs up. Participation in all aspects of Project Safeguard is voluntary, and a Veteran can withdraw from the program at any time.

Procedures

If you agree to participate in Project Safeguard, you will be connected to a Project Safeguard Community Care Navigator in your area who will be able to assist you in accessing needed services. You understand that if you agree to participate in Project Safeguard, you may be asked to fill out an initial survey so we can best understand your needs. Project Safeguard works closely with other organizations and you acknowledge that your information may be shared with these partnered organizations only to best support your reported needs. You understand that you do not have to answer any questions that you do not wish to answer.

Participation and access to requested services may require that you share certain elements of your personal data. Project Safeguard would like your express permission and consent to share the minimum personal information necessary, such as your name, address, and phone number if this information is required to connect you to the vetted partners or as needed based upon specific requests you may make. For example, a request for mental health assistance may require us to share these data elements with a service provider in order to meet your needs. We will not share your personal information for commercial purposes, such as selling email lists, advertising, or unsolicited marketing requests. If you decide to not consent, it may impact your ability to access requested services and/or information.
